the Molniya launch vehicle, lots of planetary probes launched, but
limited success. The worst case was a Mars launch on 24 Oct 1962.
During the burn of the fourth stage to leave LEO for Mars transfer,
the stage exploded. 24 fragments were catalogued. This was the only
big explosion, but a couple of the Venus launches apparently broke
apart with 7 or 8 fragments each.
