According to the article, the loss of Salyut-7 from human error was 
the 1985 incident that put the station into a deep freeze; Salyut 7 
was rescued from this condition by the crew of Soyuz T-13 and 
subsequently used by the crew of Soyuz T-15.

The event that effectively ended Salyut-7 was a failure of the Cosmos 
1686 power system in 1989, which was not caused by human error. At 
least that is what the article says.
