I will continue to argue that Soviet/Russian and American space teams have
shown that they know how to fly 'safely',
even in the face of the hazards, and that disasters that have occurred in
recent decades have been caused
NOT by the hazards inherent in space operations BUT when engineers,
operators, and managers FORGET the
lessons of keeping danger at bay, and make judgment mistakes that were
avoidable. I argue that neither shuttle
disaster was an 'accident' -- a caught-by-surprise "how-did-THAT-happen"
random event -- but were consequences
of avoidable mental flaws. Neither crew had to die to teach NASA things it
already knew -- but had forgotten.
