Tskyklon, Kosmos (the later), and Molniya have all, I believe, been launched
from Plesetsk when it was in the Soviet Union and when it was in Russia, as
well as from Baikonur (both in SU days and Kazakhstan days). Depending on
how you count that, it's either two or three countries, and I expect Molniya
may be launched from Kourou too. Vostok I think was retired before the end
of the SU, so although it flew from Plesetsk and Baikonur, it would only
count as flying from one country.
Proton has only flown from Baikonur, but that could be considered as flying
from two countries.
