[FPSPACE] Shuttle flight delayed?!

The shuttle launch is on hold as engineers examine a bolt connecting the
shuttle to the external tank.  Apparently they are concerned because a
similar bolt did not fully retract on the last mission.

Now isn't this awfully odd?  Isn't this EXACTLY the type of thing that
they are supposed to address during a Flight Readiness Briefing?  In fact,
during several of the past launches there have been last minute re-checks
of documentation and stuff like that.  Is anybody keeping track of this
stuff?  It seems awfully darned worrisome that these kinds of things are
getting through the readiness briefings and are coming up during flight
day.
